General Terms of Application and Employment
I understand that Rapid Fired Pizza’s acceptance of this application does not obligate Rapid Fired Pizza to offer me employment.
I understand that if I accept employment it will be employment-at-will, which means that I have the right to terminate my employment at any time with or without cause or notice, and that Rapid Fired Pizza reserves this same right. I understand Rapid Fired Pizza has the right to modify its policies and practices without giving me notice of the changes. I understand that after an offer of employment has been made and during my employment, Rapid Fired Pizza has the right to require a physical examination to determine my fitness and ability to work, including drug and alcohol screening tests. I will, when requested, undergo such examination/testing, and sign a consent and release statement provided by Rapid Fired Pizza. I understand that refusal to undergo such examination/testing and to sign the consent and release statement may result in termination of my employment.

Consent and Authorization to Release Information
I authorize Rapid Fired Pizza and its authorized agents to investigate all statements in this application, including my present and past work history, academic credentials and transcripts and other records, and references. I authorize Rapid Fired Pizza and its agents to obtain from such sources information concerning my character, habits and ability, or the cause of my leaving academic institutions or employment. I also authorize Rapid Fired Pizza to conduct a public records search regarding my background, including a criminal history records verification, social security number verification, employment-related credit history profile, and/or motor vehicle operator license verification, at any time as deemed necessary by Rapid Fired Pizza. I also authorize Rapid Fired Pizza to make an inquiry to a consumer-reporting agency (“CRA”) concerning my present and/or past work history. CRAs are established for the purpose of receiving, storing, and disseminating information regarding employment dishonesty, retail theft and related incidents, other acts of dishonesty, violence or drug-related offenses reported to them.
I understand I have the right to inquire as to whether any or all of the foregoing information is required for the position for which I am applying. I understand that under the Fair Credit Reporting Act I may make a timely request of a description of any credit report. Additionally, I understand that I may obtain a copy of any CRA report and may dispute the accuracy or completeness of the information reported to Rapid Fired Pizza by writing or calling the CRA at the address or telephone number provided to me at the time I receive a copy of the report.
